Welcome to AlertGem, our first product AlertCard, is an Personal Health Record. The App gives you full control over your health information and prints out a physical wallet card and information sheet containing almost your entire medical record on a waterproof, rip-proof, anti-microbial paper. We send you two of these cards, one for you and one for your primary emergency contact.

Our process makes it easy, we send you reminders to update your information every 3 months and fresh Wallet Cards and InfoSheets depending on which plan you have every 6 months.
All Your Information At A Glance
Your AlertCard not only provides First Responders and Emergency Room Staff with everything they need to treat you in a medical emergency, it also keeps all your information handy for new appointments. No more searching for information when handed a packet of paperwork, you’ll have everything you need right in your pocket. Or better yet, bring in a copy of your AlertCard InfoSheet and just have them make a copy, it has almost everything on it and will be a lot easier to read than your handwriting.
The back of your InfoSheet is dedicated to your medical information while the front covers your contact information, emergency contacts, care team, and the people and pets you care for, among other things. Both the Wallet Card and InfoSheet are printed on a unique waterproof, rip-proof, anti-microbial paper.
There will be a few questionnaires that you may need to fill out, like the PHQ-9 for Depression or your Pain Scales. We can’t answer everything for you, but once you’ve entered it into your AlertCard record, you’ll be able to print out a copy and hand it to your doctors.
Accessible through all devices
The AlertCard Online Personal Health Record App works on all devices and your information is available online and offline. Your Wallet Cards and InfoSheets mean your Medical Information is available to First Responders and ER Staff in an emergency even when the power phones are out
